May - June 2018 At Hydroil and Proline, was urgently contacted to provide a comprehensive Incident Report for SafeWorkSA after a catastrophic incident, then over two weeks risk assessed high risk plant, high risk hazardous substances, staff duties and associated Hazardous Manual Tasks. Given there were many risks with a residual risk rating above moderate, (upon the Hazard Register), whilst supporting the hierarchy of controls high standard Safe Operating Procedures (SOPs) were provided. WHS High standard, end on end combined WHS Policies and Procedures were then uploaded to support inspections, inductions, and schedules. The third and subsequent weeks required Worker Engagement upon consultation and training upon. The new SOPS were formulated based upon existing SOPS and were then edited to meet internal and external businesses.

July 2018 At Nicks Crane Services (NCS), over two weeks Risk Assessed 24 high risk plant, 23 high risk hazardous substances, 9 staff duties and 9 associated Hazardous Manual Tasks. Given there were many risks with a residual risk rating above moderate, (upon the Hazard Register), new high standard SOP and SWMS were provided, whilst supporting the hierarchy of controls. WHS High standard, end on end combined WHS Policies and Procedures were then uploaded to support inspections, inductions, and schedules. The third and subsequent weeks required Worker Engagement upon consultation and training upon. The new Safe Work Method Statements (SWMS) were formulated based upon existing SWMS and were then edited to meet internal and external businesses. The total WHS systems was planned to ensure it was systematic and pragmatic e.g. one SWMS per project or activity e.g. Crane and Slinging work, and not several SWMS for each step activity within Crane and Slinging work.

Of interest and a non NCS incident, but still involved their Crane truck with lifting capacity of over 80 tonnes, was ‘insurance written off” after a serious Contractor controlled WHS incident at Crafers in December 2017. In response, the relevant NCS Plant Assessment had ample content of identified risks, assessments and controls, and the associated SWMS had high consequent statements to prevent a reoccurrence, followed up with high consequent Operator consultation and training engagement Safe Work Method Statements and Safe Operating Procedures.
